Instructions for filling out form:

A cheque or money order, or bank draft payable to Quinndale Community Council,
NSF cheques are subject to a $ 10 Fee.
No post dated cheques accepted
First time registrants must provide a copy of a Birth Certificate or valid proof of age document
Separate registration forms and separate payments must be submitted for each player.
Completed registration forms will be processed in order of receipt. Quinndale is not responsible for undelivered mail.
No guarantees with registrations received where the cheque does not clear. You will be contacted and notified, but paid registrations will take priority.
